Resumo:Data analyzed in this paper were collected within the framework of the Lepra Evaluation Project, an epidemiological study of leprosy in Karonga District, northern Malawi. For 212 patients information on the number of skin lesions, slit-skin smear and skin biopsy results were available. Among 61 patients with a single lesion none were slit-skin-smear positive and two had bacilli detected in skin biopsies. In contrast, among 119 patients with four or more lesions 34 (28.6%) versus 59 (49.6%) had bacilli detectable in slit-skin smears or skin biopsies, respectively. In a further 47 patients skin biopsy results could be compared with split-nerve biopsy results. In 20 of 47 patients the bacterial indexes (BIs) were identical in skin and nerve biopsies, while in 26 of 47 patients the BIs were higher in nerve than in skin biopsies. This difference, which is consistent with several other studies in the literature, provides an insight into the pathogenesis of leprosy. (AU)^ien.

Título:Sensitivity and specificity of methods of classification of leprosy without use of skin-smear examination.

Resumo:A 12 -month cohort of 2664 new leprosy cases in Bangladesh has been analyzed to provide information about the sensitivity and specificity of two different methods of classifying leprosy into paucibaccilary (PB) and multibaccilary (MB), if the results of skin-smear examination are note taken into account. The two methods are: 1) a procedure based on counting skin lesions recommended by the World Health Organization (WHO) (<6 skin lesion = PB, > 6 skin lesions = MB); and 2) the "Bangladesh method" (<10 skin and nerve lesions = PB, > 10 skin and nerve lesions = MB). In the latter system, any degree of nerve enlargement is taken to be a nerve lesion. The WHO method was found to be 89% sensitive and 88% specific at detecting smear-positive MB cases frm among the cohort; the Bangladesh system, 92% sensitive and 88,6% specific. The WHO method did not detect 18 smear-positive cases as MN; the Bangladesh method left 13 smear-positive cases unclassified as MB. Several of these "missed" (false-negative) cases had a high bacterial index. The WHO system of classifying leprosy cases a MB is simple to appply and has a reasonable balance between sensitivity and specificity. However, it must be recognized that the system will lead to a small but significant number of skin-smear-positive MB cases being treated with a PB treatment regimen. (AU)^ien.

Resumo:Two of the Mycobacterium leprae-specific assays--a serum antibody competition (for an epitope on 35-kDa protein) test (SACT) and an enzyme-linked immunosorbent assay (ELISA) for the disaccharide epitope of phenolic glycolipid-I (PGDS)--were comparatively evaluated as tools for monitoring chemotherapy in 125 lepromatous leprosy (LL/BL) patients. An adaptation of the SACT from a radioimmunoassay (RIA) to an ELISA procedure is also described. A moderate but statistically significant correlation was observed between the assays, although SACT appeared to be the more sensitive of the two. Levels of antibodies correlated better with the bacterial index (BI) than with the duration of treatment. However, wide individual variations in antibody levels (for a specific duration of treatment or BI) were seen in treated as well as untreated patients. Anti-PGDS antibody response of the IgG type was poorer than that of the IgM type and, apparently, it did not have a bearing on either treatment duration or the BI. Further studies will be needed to clarify whether the treated patients showing a negative (or low) BI and high antibody levels were harboring hidden foci of active infection, and whether treatment could safely be terminated in patients showing low values for both BI and antibody^ien.

Resumo:Three case reports of patients with a single, nodular, subpolar lepromatous skin lesion, one on the left elbow, another on the posterior aspect of the left leg, and the third on the extensor ulnar aspect of the right forearm, are presented. The lesions, clinically and histopathologically, resemble lepromas which develop at the site of experimental inoculation of Mycobacterium leprae in armadillos. These are sites on the body which are likely to be traumatized. With the distinct possibility of the presence of viable M. leprae in the soil of Louisiana and Texas from wild armadillos with the natural disease, it is suggested that these three patients acquired the infection from the environment and had inoculation lepromas^ien.

Resumo:A polyclonal serum sample from a lepromatous leprosy (LL) patient, which presented a specific recognition pattern for leprosin, was used to screen a Mycobacterium leprae genomic library constructed with DNA isolated from human lepromas. One clone, designated ML4-1, which expressed a specific antigenic determinant of M. leprae as part of a beta-galactosidase fusion protein, was isolated. The 1.932 bp M. leprae-derived genomic fragment was sequenced, and it had an incomplete open-reading frame shown to code for a 644 amino-acid polypeptide (72.3 kDa). Some partial nucleotide homology to the M. tuberculosis MTCY9C4 cosmid and the M. leprae B1913 cosmid were found. Southern blot assays using the 584 bp Eco RI-Bam HI fragment excised from the ML4-1 clone revealed that this sequence is present only in the M. leprae genome and not in the 24 different mycobacterial DNA tested. Two oligonucleotides based on the genomic sequence were also synthesized and used as amplifiers for a polymerase chain reaction (PCR) test, giving a positive signal exclusively in M. leprae DNA. Furthermore, 32 sequential synthetic peptides, 20 amino-acids long, spanning the entire protein corresponding to the hypothetical ML4-1 clone sequence, were synthesized and evaluated by ELISA. A peptide included in the 221-240 region was significantly recognized by either lepromatous leprosy or healthy tuberculosis contact patient sera. Thus, PCR amplification of this fragment, along with the recognition of its protein sequence by leprosy patient sera, could be a useful tool for a potential diagnostic method in the detection of M. leprae infection in the future. Resumo:Ultraviolet (UV) light is recognized as a potent sterilizing aid, but its relative effectiveness against Mycobacterium leprae has not been shown. We examined the influence of UV on the growth and metabolic activity of M. leprae harvested fresh from foot pads of nude mice. Temporary static suspensions were exposed to timed intervals of UV radiation generated from a fixed source to constitute dosages ranging from 0-12.64 x 10(4) erg/cm2. The metabolic activity of the bacilli was indexed by the oxidation of 14C-palmitate in BACTEC 12-B vials. The long-term effects of irradiation on cell division and growth were assessed by inoculation of BALB/c mouse foot pads. The metabolic activity in BACTEC showed an immediate dose-response-related decline to a maximum of 50% of the control activity after exposure to 6.3 x 10(4) erg/cm2. Mouse foot pad studies showed a similar dose-response pattern. Effective-dose determinations based on metabolic or foot pad data were similar. UV doses of 3.52 x 10(4) erg/cm2 resulted in an average 50% killing, and 7.73 x 10(4) erg/cm2 killed 84% of the M. leprae exposed. This UV sensitivity is similar to that reported for M. tuberculosis. UV sterilization and disinfection practices suitable for M. tuberculosis are likely to be equally effective for M. leprae. (AU)^ien.

Resumo:Several discoveries about leprosy indicate that Mycobacterium leprae transmission mainly accurs by inhalation, and the nose is major port of entry and exit. The present study evaluated the clinical application of PCR for detection of M. leprae DNA in nasal mucosa biopsies in untreated leprosy patients (52) and their contacts (99) from the State Reference Center in Sanitary Dermatology and Leprosy, Uberlandia, MG, Brazil. PCR detection of a 372-base pair DNA fragment from M. leprae was accomplished in 36 (69.2%) patients, from which 34 (91.9%) of them were multibacillaries. Furhermore, PCR was positive in 3 (16.7%) of 18 slit-skin smear negative, 4 (25.0%) of 16 skin lesion BI negative, 8 (33.3%) of 24 nasal mucosa BI negative patients, and 10 of 99 contacts (10.1%). The presence of bacilli in 10.1% of the contats may potentially reflect an occult leprosy, and reflect an occult leprosy, and these patients must be accompanied, followed by a chemoprophylaxy treatment. Considering all PCR results against clinical and BI classification of patients and controls, we have found a sensitivity of 69.2%, a specificity of 89.9% and an accuracy of 82.8%. It has been demonstrated here through PCR of nasal biopsies that the bacillus invades the mucosa, passing through the nasal inferior turbinate to reach peripheral blood. Therefore, the molecular investigation of invasive nasal biopsies by PCR tests has proven to be useful in defining patients of higher risk of transmission and risk-group contacts, which is an important step ti reach the World Health Organization objective towards the elimination of leprosy as a public health problem (AU).

Resumo:Out of 265 biopsies of leprosy patients received at the Experimental Laboratory of Schieffelin Leprosy Research and Training Centre from 1987 to 1997 for evaluating resistant strains of M. leprae using the mouse footpad technique, 49 showed resistant strains of M. leprae to varying concentration of dapsone, rifampicin and clofazimine. 23 (47%) of these were from a control area. With 369 skin-smear positive multibacillary (MB) patients as the risk group (denominator), 23 (6.23%) were resistant to one or more drugs. 18 (4.88%) had dapsone resistance, 5 (1.36%) were resistant to rifampicin and 9 (2.44%) had resistance to low concentrations of clofazimine (0.0001%). Out of the 23 biopsies with drug resistance from the control area, primary dapsone resistance was seen in 7 (30%) biopsies and secondary dapsone resistance in 11 (48%). Primary rifampicin resitance was seen in 4 (17.4%) patients, secondary rifampicin resistance in 1 (4.35%) and primary clofazimine resistance in 7 (30%). 3 (13%) of the strains showed secondary clofazimine resistance. One biopsy had resistent strains to all the three drugs. In a control area where properly supervised effective multidrug therapy (MDT) was regularly administered over the years, the emergence of drug resistance is negligible. It may not be the case if the content, duration and regularity of the drug regimen were not satisfactory. Aware of the possible shortcomings in mass administration of MDT, it is emplasized that mouse footpad studies on drug resistance should be made available at least in endemic areas where the incidence of the disease has not changed despite good MDT coverage in order to monitor the emergence of drug resistance. Research into molecular biological identification of drug resistant-M.leprae should be intensified. These steps would help to institute timely measures to check the spread of any drug-resistant organisms in the community (AU).

Resumo:In sumary, it may well be conculuded that the nude mouse with thymus cell grafting as a severe lepromatous leprosy model was developed in C57BL/6-nu/nu as well as CF#1-nu/nu, comparing with untreated nude mice. In addtion, comparison of sex in treated group, susceptibility to M. leprae in the some males were higher than in foot swelling of the females, also, difference of individuality occured in both sex at a latter stage after infection, therefore, further investigation is under way to look for preferable nude mouse and hairless-athymic mouse which they are established by ourselves.

Resumo:El armadillo de 7-bandas, Dasypus hybridus, ha sido utilizado en estudios de lepra experimental en un reducido numero de animales. Con el proposito de conocer la susceptibilidad de esta especie frente al Mycobacterium leprae y de llevar a cabo estudios patologicos con microscopia de luz y microscopia electronica se inoculo cinco armadillos con germenes provenientes de lesiones lepromatosas de dos enfermos sin tratamiento. Se encontro lesiones lepromatosas disemidas en uno solo de los cinco armadillos inoculados las que morfologicamente son similares a las previamente descritas por otros autores en esta misma especie, en D. novemcinctus y D. sabanicola. Se describe aspectos ultraestructurales del granuloma lepromatoso y del M. leprae. Se describe tambiem lesiones degenerativas de fibras miocardicas y de fibras musculares esqueleticas en el armadillo que presento lesiones lepromatosas, asi como en los cuatro armadillos en los que esta no se desarrollaron y se discute la natureza de estas lesiones y de otras de tipo granulomatoso inespecifico en la serosa pleural y peritoneal (AU).
